Authorization to release information form
While private health information is explicitly protected under HIPAA, there are ways for patients to provide authorization to release information of their personal health records to a third party. This can be necessary for several reasons, such as medical insurance to see the price of treatments or a scientist for the purposes of research of a survey.
These parties do not typically have access to these documents, but they are able to view them for a limited amount of time if the patient signs an authorization to release information. This document is prepared by the responsible healthcare provider. HIPAA requires the release form to be written in clear English so that the patient has no misunderstanding as to what he or she is agreeing to.
This allows patients to release information in a way that is most appropriate for each situation as it arises. Patients are able to decide the individual portions of their medical records that are shared, with whom those records are being shared, and for what length of time that those records are available.
