The healthcare provider is responsible to draft and produce a specialized release form that details the nature of the information being released, to whom that data will be available, and the duration of time that party will have access to the patient’s private health information.

In the case of paper medical release forms, many are needed to be converted back to electronic documents after being signed by the patient. The medical release of information form is crucial for any entity to keep readily available on file.
